Tubal blockage due to infection and associated complications
Patent but non functional tubes
Previously sterilized and in those whom recanalisation is not feasible

2. Unexplainod infertility
Age of the women is more than 36 years, NF is an earlier option
Following six or more failures with intrauterine
3. Polycystic ovarian disease
If there is persistent elevation of LM, failure of six or more attempts of intrauterine insemination or following a poor response to conventional ovulation inducing agents
4. Endometriosis
In cases of severe endometriosis and in those cases of mild to moderate where conventional treatment regimes have failed (as in unexplained)
5. Cervical factor/Immunological infertility
Presence of Antisperm antibodies either in the cervical mucous or in serology

Azoospermia – if the couple are unwilling for therapeutic donor insemination
Any congenital conditions requiring sperm retrieval procedures from the epididymis or testis
